clinic-ready — 1L IO/IO vs IO/TKI by IMDC; no head-to-head among doublets in this pack

Evidence page

Who this applies to

  • Metastatic RCC; record histology (clear vs non-clear), sarcomatoid, IMDC risk (KPS, time from diagnosis to systemic therapy, hemoglobin, calcium, neutrophils, platelets), prior nephrectomy, prior adjuvant PD-167.

Standard options

Clinical contextOptionKey inclusionTypical course
IMDC intermediate/poor risk, durability/complete-response priorityNivolumab plus ipilimumab1Clear-cell advanced RCC and immune-therapy candidacy1.Induction followed by nivolumab maintenance in CheckMate 2141.
Any IMDC risk with need for response and an acceptable TKI partnerPD-1/VEGFR combination234Balance disease burden, organ function, hypertension, and toxicity profile67.Regimen-specific continuous TKI plus PD-1 schedule234.
After ICI and antiangiogenic therapyBelzutifan or other sequence matched to prior exposure5LITESPARK-005 population and local label5.Regimen-specific5.
Non-clear-cell or atypical clinical contextTrial or individualized systemic approach67Do not apply clear-cell trial effect estimates unchanged67.Individualized6.
  • IMDC intermediate/poor, goal durability / CR: nivo 3 mg/kg + ipi 1 mg/kg q3wk ×4 then nivo (CheckMate 214)1. Higher-grade irAE than TKI monotherapy; lower grade 3–4 TRAE than sunitinib in the primary paper1.
  • Any IMDC, need response / TKI partner acceptable: pembro + axitinib (KEYNOTE-426)2, nivo + cabo 40 mg (CheckMate 9ER)3, or pembro + lenvatinib 20 mg (CLEAR)4. Hypertension, diarrhea, hand-foot, hepatic toxicity234.
  • Do not add cabo to nivo/ipi as default (COSMIC-313: no OS gain, more grade 3/4 TRAE)67.
  • Later line after VEGFR and ICI: belzutifan vs everolimus (LITESPARK-005) improved PFS/ORR not OS5. Nivo vs everolimus (CheckMate 025) is historical post-VEGFR6. TiNivo-2: adding nivo to tivozanib after ICI did not improve PFS67.
  • Local therapy (CN, MDT) is individualized — not covered by these systemic trials67.

Therapy sequencing

Starting point or prior exposureNext treatment orderBranch point / evidence boundary
Untreated IMDC intermediate/poor-risk ccRCC, priority on durable responseNivolumab plus ipilimumab induction → nivolumab maintenance1.This is an IO/IO pathway; it is not a direct comparison with each PD-1/VEGFR doublet16.
Untreated ccRCC, need for response or acceptable VEGFR partnerStart pembrolizumab/axitinib, nivolumab/cabozantinib, or pembrolizumab/lenvatinib234.Select for disease burden, hypertension/proteinuria, liver function, and immune risk; no head-to-head doublet trial in this page identifies a universal preferred regimen67.
Progression after both PD-1/PD-L1 and antiangiogenic therapyBelzutifan5.LITESPARK-005 directly studied this exposure sequence; its OS endpoint was not statistically significant at the reported analysis5.
Progression after IO/IO without prior VEGFR therapyChoose a VEGFR-containing option before positioning belzutifan56.Belzutifan's direct pivotal population had prior ICI and antiangiogenic treatment; this page does not establish a single preferred post-IO/IO sequence56.
Considering nivolumab/ipilimumab/cabozantinibDo not add cabozantinib to IO/IO as default67.COSMIC-313 improved PFS but not OS and increased grade 3/4 treatment-related AEs67.

Surveillance and treatment de-escalation

Response-assessment imaging schedule

PhaseImaging cadenceStop, hold, or de-escalateRe-escalation / next line
Active first-line IO ± TKI (response assessment)67Cross-sectional imaging every 8–12 weeks while assessing response; CBC, CMP, TSH, blood pressure, urine protein; immune and TKI toxicity review each cycle67.Continue until progression or toxicity; nivo/ipi induction completes 4 cycles then nivo maintenance; TKI partners continue with PD-1 until progression unless toxicity mandates switch1234. Hold TKI for hypertension, hand-foot, or hepatic toxicity; hold IO for immune events234.Progression after ICI and VEGFR → belzutifan (LITESPARK-005 population) or trial; avoid default nivo/ipi/cabo (COSMIC-313 no OS gain)56.
Prolonged response / durable disease control67After prolonged response, extend imaging intervals beyond 8–12 weeks when clinically appropriate per EAU/NCCN follow-up frameworks67.Treatment breaks are not trial-established for IO/TKI doublets; individualize only with shared decision67.Confirmed progression → next-line sequencing56.
  • Cytoreductive nephrectomy and metastasis-directed therapy are individualized and not governed by these trial surveillance schedules67.

Biomarkers

MarkerCurrent clinic roleEmerging / investigationalHow it changes decision nowLimits
Histology (clear-cell vs non-clear)Primary frame for applying 1L doublet evidence67Non-clear-cell randomized first-line landscape still thinner than clear-cell67Clear-cell: use IO/IO or IO/VEGFR doublets as on this page; non-clear: trial or individualized approach without transplanting clear-cell effect sizes67.Sarcomatoid features may favor IO-containing regimens but do not replace IMDC/histology counseling67.
IMDC riskPractical 1L selector (especially for nivo/ipi intermediate/poor)167—Intermediate/poor: durability/CR discussion often favors nivo/ipi; any-risk response need may favor PD-1/VEGFR1234.Does not rank IO/VEGFR doublets head-to-head67.
PD-L1Not the primary selector for these doublets12Exploratory correlatives in 1L ICI trials12Does not choose among approved doublets in routine clinic12.No validated threshold for regimen selection on this page12.
Angiogenesis / T-cell expression signaturesNot used for routine regimen choice2KEYNOTE-426 exploratory: TcellinfGEP and angiogenesis signatures associated with outcomes2Does not change first-line doublet selection today2.Exploratory analyses only; no practice-ready cutoffs on this page2.
Germline (VHL, FH, etc.)Consider for non-clear histology or hereditary-syndrome features69Broader hereditary panels when indicated69Informs genetic counseling and selected targeted pathways; not required to start 1L IO doublets in sporadic ccRCC69.Does not replace IMDC or histology for 1L IO selection67.

Upcoming research

Field direction: first-line IO doublets remain the core; HIF-2 (belzutifan) is moving earlier and into combinations, adjuvant pembrolizumab exposure changes relapse counseling, and non-clear-cell randomized first-line data remain thinner than clear-cell567.

Program or themeWhy it may change careEvidence status
Relapse after adjuvant pembrolizumab (KEYNOTE-564)Changes subsequent IO/VEGFR and trial eligibility counseling67.Adjuvant SOC established; relapse-path sequencing still evolving in practice67.
Earlier / combination HIF-2 (belzutifan)Could move HIF-2 ahead of current later-line use56.Later-line peer-reviewed/label evidence; earlier combinations not a 1L default in this pack56.
Non-clear-cell randomized first-line landscapeWould define histology-specific doublets67.Not retrieved as a dedicated landscape on this page67.
COSMIC-313 tripletTriplet did not improve OS versus doublet in retrieved sources67.Negative OS for the triplet; not a care-changing upgrade67.

Epic phrase

#Metastatic renal cell carcinoma

  • Counseling: CheckMate 214 nivolumab/ipilimumab in IMDC intermediate/poor risk: 18-month OS about 75% vs 60% with sunitinib (OS HR 0.63 vs sunitinib). KEYNOTE-426 pembrolizumab/axitinib 12-month OS about 90% vs 78% with sunitinib (interim OS HR 0.53 vs sunitinib). CheckMate 9ER nivo/cabo PFS HR 0.51 vs sunitinib. No head-to-head comparison ranks doublets. Later belzutifan improved PFS vs everolimus after prior ICI and VEGFR therapy; OS was not statistically significant at reported analysis.
  • Counseling: Reviewed surveillance and de-escalation: imaging every 8–12 weeks during active response assessment on first-line therapy; after prolonged response, extend imaging intervals per EAU/NCCN when clinically appropriate. Complete nivo/ipi induction then maintenance; hold TKI/IO for toxicity; progression after ICI and VEGFR → belzutifan or trial; treatment breaks for doublets are not trial-established.
  • Monitoring: For IO/IO, review bowel, lung, liver, skin, and endocrine symptoms before each infusion. For IO/TKI, also check blood pressure, urine protein, liver tests, diarrhea, and hand-foot/skin effects. For belzutifan, check CBC and oxygen saturation. Promptly contact oncology for diarrhea/abdominal pain, cough/dyspnea, rash, jaundice, marked fatigue, hypertension symptoms, hand-foot/skin changes, or anemia/hypoxia symptoms. *** after-hours/urgent-assessment instructions reviewed.
