clinic-ready overview — SCD gene therapy and heme CAR-T as labeled SOC where indicated; solid-tumor cellular mostly investigational outside melanoma TIL

Evidence page

Who this applies to

  • Clinic orientation across hematology and oncology when counseling FDA-approved gene or cellular products (sickle cell disease gene therapy; autologous CD19/BCMA CAR-T; melanoma TIL) or referring to investigational CAR-T/TCR-T/TIL/gene-therapy trials, including GU solid tumors.12345
  • Not a substitute for disease-specific heme or solid-tumor SOC pages; radioligands (e.g. Pluvicto), ADCs, and small molecules stay on those disease pages.6
  • Confirm product label / trial eligibility (antigen, HLA, prior lines, organ function, donor availability, manufacturing logistics) before counseling as SOC or trial.1234

SCD with matched related donor / allo-HCT candidateAllogeneic HCT (separate modality from autologous gene therapy)1237Pivotal Casgevy/Lyfgenia designs excluded patients with an available 10/10 HLA-matched related donor; allo-HCT remains the alternative potentially curative path when a suitable donor exists1237Myeloablative or reduced-intensity allo conditioning + donor graft; GVHD/infection/VOD prophylaxis; not gene-add or gene-edit of autologous cells37

SCD gene therapy — mechanism, manufacturing, and conditioning counseling

Both FDA-labeled SCD gene therapies are one-time autologous hematopoietic stem-cell products that require cell collection, ex vivo genetic modification, myeloablative busulfan conditioning, and inpatient transplant-level supportive care (including blood-product support) until engraftment.12

StepWhat to counselProduct notes
Shared platformRisks/benefits/alternatives of high-dose chemotherapy followed by infusion of genetically modified autologous HSCs; regimen-related morbidity/mortality; short- and long-term chemo consequences (including infertility); need for transfusions and other supportive care during aplasia; back-up unmodified rescue CD34+ cells in case of engraftment failure or product compromise12Same transplant-like pathway for Casgevy and Lyfgenia12
Pre-collection transfusionScheduled transfusions / erythrocytapheresis before mobilization to lower HbS and stabilize Hb per label targets (product-specific Hb/HbS goals)12SCD: plerixafor mobilization — do not use G-CSF for Casgevy SCD mobilization1
Manufacturing — Lyfgenia (lovo-cel)Autologous CD34+ cells transduced ex vivo with BB305 lentiviral vector encoding a modified β-globin gene (βA-T87Q-globin); after engraftment, RBCs can produce anti-sickling HbAT87Q that decreases the proportion of HbS, with the goal of reducing sickled RBCs, hemolysis, and VOEs2One-time IV dose; clinical program includes HGB-206 (NCT02140554), phase 3 HGB-210 (NCT04293185), and long-term follow-up LTF-307 (NCT04628585)2383940
Manufacturing — Casgevy (exa-cel)Autologous CD34+ cells edited ex vivo with CRISPR/Cas9 at the erythroid-specific BCL11A enhancer to reduce BCL11A expression and increase fetal hemoglobin (HbF); higher HbF lowers intracellular HbS and addresses VOC biology; also labeled for TDT17One-time IV dose after busulfan; CLIMB SCD-121 / long-term follow-up pathway17
ConditioningPK-adjusted (or label-directed) myeloablative busulfan; consider seizure prophylaxis (avoid phenytoin) and VOD/SOS prophylaxis; remain inpatient through engraftment12Delayed platelet engraftment and mucositis/febrile neutropenia are expected transplant toxicities12

Casgevy SCD (Trial 1 / label)17SCD with recurrent severe VOCs17≥2 severe VOCs each of 2 years pre-screening (pivotal); age ≥2 on current US label17Available 10/10 HLA-matched related donor; advanced liver disease; untreated/high-risk Moyamoya; chronic-pain ED/hospitalization pattern as in protocol17
Lyfgenia Study 1-C / label2SCD with VOE history2Age ≥12; efficacy set ≥4 VOEs in 24 mo; busulfan conditioning required2α-thalassemia trait (−α3.7/−α3.7) anemia/dysplasia limitation; not studied with >2 α-globin gene deletions; willing matched sibling donor excluded in pivotal design2
ZUMA-7 / TRANSFORM (2L LBCL)8910Early R/R LBCL8910Primary refractory or relapse ≤12 mo after 1L8910BELINDA (tisa-cel vs SOC) did not show EFS superiority — tisa-cel is not a 2L early-failure label pathway from that trial323
KarMMa-3 / CARTITUDE-41415Lenalidomide-exposed / triple-class–exposed RRMM as defined in each protocol1415Prior-line windows per trial (ide-cel 2–4 prior; cilta-cel 1–3 prior, len-refractory)1415Do not import cross-trial ORR as head-to-head product ranking without matching3

Therapy sequencing

Starting pointPreferred orderEvidence boundary
SCD with recurrent severe VOCs / VOEs eligible for autologous gene therapyDiscuss Casgevy vs Lyfgenia at a qualified center after hydroxyurea/disease-modifying therapy counseling; both require myeloablative busulfan, fertility planning, transfusion support through aplasia, and rescue-cell planning; neither has a randomized comparator arm127Product choice is label-, center-, and risk-profile–driven (e.g. Lyfgenia hematologic malignancy boxed warning); not ranked head-to-head on this page12
SCD with available matched related donorCounsel allo-HCT vs continuing disease-modifying therapy vs (if still eligible) autologous gene therapy only when donor path is declined/unavailable per center practice; pivotal gene-therapy trials excluded 10/10 matched related donors1237Allo-HCT adds GVHD and graft-failure risk not present with autologous gene therapy; gene therapy still carries busulfan/conditioning and (for Lyfgenia) insertional/hematologic malignancy risk1237
Early R/R LBCL (primary refractory or ≤12 mo)Prefer axi-cel or liso-cel over salvage chemo → ASCT when fit for CAR-T and label-eligible (ZUMA-7 / TRANSFORM)8910Tisa-cel BELINDA did not improve EFS vs SOC in this window23
RRMM after 1–4 prior lines (triple-class / len-refractory contexts)Prefer labeled ide-cel or cilta-cel over continuing SOC triplets when KarMMa-3 / CARTITUDE-4–like criteria met141516Exact line and regimen gates are label-specific and evolving316
Melanoma after PD-1 failureConsider lifileucel when label criteria are met517Not a GU indication5
GU solid tumor progressing on disease-setting SOCStay on the disease page for next systemic line; offer gene/cellular trial referral only as investigational619202122Do not substitute investigational CAR-T/TCR-T for labeled GU SOC6

Casgevy / Lyfgenia engraftment period12Frequent ANC and platelet counts until engraftment; bleeding precautions until platelets recover; consider VOD prophylaxis with busulfan conditioning12Rescue CD34+ cells if neutrophil engraftment failure; manage mucositis/febrile neutropenia as transplant-level care12Delayed platelet engraftment → continued transfusion support / TPO mimetic per center12
Post–SCD gene therapy long-term12Lyfgenia: CBC with differential ≥ every 6 months for ≥15 years; integration-site analysis at Months 6 and 12 and as warranted2. Casgevy: enroll/continue long-term follow-up intended through 15 years after infusion1. SCD organ surveillance (CNS, renal, cardiopulmonary, pain) continues per SCD specialist practice while VOC phenotype is reassessed127Fertility counseling completed before conditioning; infertility risk is from myeloablative busulfan, not a separate gene-edit–specific fertility trial endpoint12New VOC/VOE, cytopenia, or suspected hematologic malignancy → urgent evaluation; Lyfgenia malignancy pathway includes bone-marrow workup when indicated2

Toxicity

RegimenTrial- / label-level safety signalCounseling focus
Casgevy (SCD)17After busulfan + exa-cel, most common grade 3–4 non-laboratory AEs (≥25%) include mucositis and febrile neutropenia; grade 3–4 mucositis 38/44 (86%) in Trial 1 SCD; all patients engrafted neutrophils in Trials 1/4; infertility risk from myeloablative conditioning; consider VOD prophylaxis with busulfan; transfusion/blood-product support expected during aplasia; single-arm safety (no randomized comparator)17Expect transplant-level mucositis, cytopenias, infection, and bleeding risk until engraftment; discuss fertility preservation and blood-product support before busulfan; urgent contact for fever, bleeding, jaundice, RUQ pain, or weight gain suggesting VOD12
Lyfgenia2Boxed warning: hematologic malignancy (AML in earlier manufacturing Group A; MDS in a Group C patient with α-thalassemia trait); most common grade ≥3 AEs (≥20%) stomatitis, thrombocytopenia, neutropenia, febrile neutropenia, anemia, leukopenia; stomatitis 32/45 (71%) in labeled safety table; mobilization can trigger sickle crisis SAEs; transfusion support through conditioning/aplasia; single-arm (no randomized comparator)2Lifelong cancer surveillance (CBC ≥ q6 mo for ≥15 y); fertility preservation before busulfan; same engraftment/bleeding/infection counseling as autologous transplant; counsel α-thalassemia trait limitation of use2
Allo-HCT for SCD (alternative)37Distinct from autologous gene therapy: infection, bleeding, end-organ toxicity, secondary malignancy, VOD/SOS, graft-versus-host disease, graft failure, disease recurrence, and transplant-related mortality; GVHD, infection, and VOD prophylaxis are part of standard counseling37Higher allo-specific morbidity (especially GVHD) vs autologous gene therapy; still may offer prolonged remission/cure when a suitable donor exists — center-specific risk estimates; do not invent rates here37
Autologous CD19/BCMA CAR-T (class)34242526Boxed warnings for CRS and neurologic toxicity/ICANS; prolonged cytopenias common; hypogammaglobulinemia (cilta-cel: lab IgG <500 mg/dL in 93% in CARTITUDE-1/4 pooled label description); infections; HLH-like presentations reported as a recognized CAR-T complication class; secondary T-cell malignancy warning for BCMA- and CD19-directed products34242526Urgent contact for fever, hypotension, confusion, aphasia, seizures, severe infection; plan proximity to center for ≥2 weeks; IgG/IVIG and antimicrobial prophylaxis per center42425

#GeneCellularOverview

  • Counseling: Gene and cellular therapy is a heme/onc–wide track (sickle cell gene therapy, heme CAR-T, melanoma TIL, and investigational solid-tumor cellular programs). It is separate from radioligands, ADCs, and small-molecule SOC on disease pages.
  • Counseling: SCD gene therapy visit — reviewed risks, benefits, and alternatives of high-dose chemotherapy (myeloablative busulfan) followed by infusion of genetically modified autologous hematopoietic stem cells. Discussed regimen-related morbidity and mortality, short- and long-term consequences of high-dose chemotherapy (including infertility), and the need for supportive care during transplantation including blood-product support and its risks. Reviewed back-up unmodified rescue CD34+ cells for engraftment failure or product compromise. Statistics applicable to the disease and labeled single-arm gene-therapy outcomes were reviewed.
  • Counseling: Lyfgenia (lovotibeglogene autotemcel / lovo-cel) — one-time treatment adding functional copies of a modified β-globin gene (βA-T87Q-globin) into the patient’s own HSCs via BB305 lentiviral vector so RBCs can produce anti-sickling HbAT87Q, decreasing HbS proportion with the goal of reducing sickled RBCs, hemolysis, and VOEs. Manufacturing: mobilization (plerixafor) + apheresis → ex vivo transduction → busulfan conditioning → IV infusion. Clinical program includes HGB-206 (NCT02140554), phase 3 HGB-210 (NCT04293185), and long-term follow-up LTF-307 (NCT04628585). Label single-arm Study 1-C: VOE-CR 28/32 (88%) and sVOE-CR 30/32 (94%) between months 6–18 (no randomized comparator). Boxed warning for hematologic malignancy — CBC at least every 6 months for ≥15 years and integration-site testing at months 6 and 12. α-thalassemia trait limitation of use.
  • Counseling: Casgevy (exagamglogene autotemcel / exa-cel) — personalized ex vivo CRISPR/Cas9 editing of autologous HSCs at the erythroid BCL11A enhancer to boost fetal hemoglobin (HbF), then return of modified cells after busulfan. Also labeled for transfusion-dependent β-thalassemia. SCD pivotal single-arm Trial 1: VF12 29/31 (93.5%) free of severe VOCs ≥12 months; HF12 30/30 (100%) free of severe-VOC hospitalization ≥12 months (no randomized comparator). Do not use G-CSF for SCD mobilization.
  • Counseling: SCD gene-therapy toxicities / management — expect mucositis/stomatitis, febrile neutropenia, prolonged cytopenias, infection and bleeding until engraftment; consider VOD/SOS prophylaxis with busulfan; fertility preservation before conditioning; inpatient transplant-level monitoring then frequent CBC until recovery. Grade 3–4 mucositis about 86 in 100 (38/44) with Casgevy Trial 1 SCD; grade ≥3 stomatitis about 71 in 100 (32/45) in Lyfgenia labeled safety table (single-arm).
  • Counseling: Allo-HCT alternative for SCD — when a suitable donor exists, allogeneic transplant can offer prolonged remission/possible cure but with higher modality-specific morbidity than autologous gene therapy, including infection, bleeding, end-organ damage, secondary malignancy, VOD, graft-versus-host disease, graft failure, relapse, and transplant-related mortality. Reviewed GVHD, infection, and VOD prophylaxis. Pivotal Casgevy/Lyfgenia trials excluded patients with an available 10/10 HLA-matched related donor.
  • Counseling: LBCL early failure (primary refractory or relapse ≤12 months) — ZUMA-7: axi-cel median EFS 8.3 months vs 2.0 months with salvage chemoimmunotherapy ± ASCT; ORR 83% vs 50% and CR 65% vs 32% with that SOC; longer follow-up median OS not reached vs 31.1 months with SOC. TRANSFORM: liso-cel median EFS 10.1 months vs 2.3 months with salvage→ASCT SOC. BELINDA: tisa-cel did not improve EFS vs SOC in this window.
  • Counseling: Myeloma — KarMMa-3: ide-cel median PFS 13.3 months vs 4.4 months with physician’s-choice SOC regimens; ORR 71% vs 42% and CR 39% vs 5% with SOC. CARTITUDE-4: cilta-cel PFS HR 0.26 vs DPd/PVd SOC; 12-month PFS about 76 in 100 vs 49 in 100 with SOC; ORR 84.6% vs 67.3% and ≥CR 73.1% vs 21.8% with SOC.
  • Counseling: Other labeled heme CAR-T often rest on single-arm pivotal ORR — say so explicitly when no randomized comparator. CAR-T toxicities: CRS/ICANS (daily monitoring ≥7 days; stay near center ≥2 weeks); prolonged cytopenias; infections; hypogammaglobulinemia (IVIG when IgG <400 mg/dL per cilta-cel-style label guidance); HLH-like presentations; secondary T-cell malignancy warning. *** after-hours/urgent-assessment instructions reviewed.
  • Counseling: GU solid tumors — no approved CAR-T/TCR-T/TIL SOC on this wiki. Investigational examples: SURPASS NCT04044859; PSMA/STEAP1 CAR-T (e.g. NCT03089203, NCT06236139); TRAVERSE/ALLO-316 NCT04696731; AB-2100 NCT06245915; BNT211-01 NCT04503278.
  • Decision: *** Casgevy / Lyfgenia referral / allo-HCT evaluation if donor available / continue disease-modifying SCD therapy / heme CAR-T if label criteria met / investigational cellular trial / supportive care.
  • Labs/NGS: *** CBC with differential, type and screen / transfusion plan, infectious disease screen before apheresis, pregnancy test if applicable, IgG if CAR-T, HLA typing if allo-HCT considered.
  • Procedures: *** mobilization/apheresis, fertility preservation, central line, busulfan conditioning admission.
  • Meds: *** hydroxyurea hold timing per product; VOD prophylaxis plan; infection prophylaxis; GVHD prophylaxis if allo-HCT.
  • Referrals: *** SCD gene-therapy / transplant center, fertility preservation, social work, trial coordinator.
  • Follow-up: *** (engraftment labs; Lyfgenia CBC q6 mo ≥15 y + integration-site mo 6/12; SCD organ monitoring; allo-HCT GVHD clinic if applicable).
